If we say we have no sin, we deceive ourselves, and the truth is not in us. If we confess our sins, he is faithful and just to forgive us our sins, and to cleanse us from all unrighteousness. If we say that we have not sinned, we make him a liar, and his word is not in us.
MY little children, these things write I unto you, that ye sin not. And if any man sin, we have an advocate with the Father, Jesus Christ the righteous: And he is the propitiation for our sins: and not for our sins only, but also for the sins of the whole world.

There is nobody that can honestly say they have never sinned. Before we got saved, we were slaves to sin. After getting saved, even though we are supposed to put away willful transgressions, the best Christians will still fall short of God's holiness through sins they didn't mean to commit or didn't realize they had committed. Thankfully, those sins done by mistake are under the blood atonement. When they are brought to our attention, then we should make confession. While willful sins are not under the original blood covering, as it says in Hebrews 10:26, we know that if we confess those sins, God is faithful and just to forgive them, and cleanse us of all unrighteousness.
The main thing I get from this passage is how we need to make it a regular practice to confess our sins to God. We should always choose not to commit sins when we are faced with a clear cut choice to do right or wrong, but what about those things we do without meaning to, like bad thoughts or sinning in our anger? Sometimes we don't even realize we have done anything wrong. Sometimes God convicts us immediately. In the cases where God shows us our sin, we should immediately confess them. We should also make it a practice to ask for forgiveness for sins we did and didn't realize. John is telling us that he doesn't want us to sin. He is writing to us that we sin not. When we do sin, Jesus acts as our mediator with the Father, so it is not necessary to confess to a human Priest. We can go straight to Jesus.
